Case Management Program -

One-on-one help in creating a better life

Imagine for a moment that you suddenly became homeless.  

Perhaps you are fortunate enough to have a small bag of your belongings with you, perhaps not. The police have referred you to an overnight shelter where you are greeted warmly by friendly faces. You are given a bed and a meal and slowly begin to move beyond your immediate concern for how you will survive through the night. "What next?" you ask yourself.

Here is where our case managers come in. Our trained case managers introduce themselves to our guests and work with them one-on one to solve the problems that are keeping them homeless. Moving beyond homelessness is complicated because most of our guests have more than one difficulty. Often, these are the same difficulties that caused their homelessness in the first place.

Our case managers help our guests every step of the way. They work with our guests one-on-one to help them:

  • find housing

  • identify needs

  • set goals

  • establish priorities

  • find service providers that can help

  • determine eligibility requirements

  • determine availability of appropriate programs

  • set appointments

  • complete applications for various programs, and

  • identify the next problem or issue to be worked on.

In 2009, our case managers made at least 9,021 contacts with 1,357 different guests. They discussed 15,668 distinct issues, which included...

  • employment (discussed 1,817 times)

  • permanent housing (discussed 1,465 times)

  • medical care (discussed 1,287 times)

  • food (discussed 1,162 times)

  • substance abuse treatment

  • mental health care

  • clothing

  • transitional shelter

  • furniture

  • job skills training

  • dental care

  • AIDS services

  • public aid and veterans benefits

  • obtaining identification cards, and

  • transportation to other programs.
